    JOB OVERVIEW:

    The Volunteer II provides companionship and support to terminally ill patients in SNF, AFC, ALF and other related facilities.

     

    My commitment will include:

    • Providing companionship and support to patients and families through support activities as indicated through the volunteer care plan.
    • Documenting services provided and pertinent findings during each visit.
    • Noting and reporting significant changes in the condition or environment and patient to the skilled professional in accordance with agency policy.
    • Following all environmental care and safety guidelines and standards per agency protocol.
    • Clearly articulating the hospice philosophy.
    • Providing an environment which promotes respect for patient, privacy, and property.
    • Demonstrating knowledge of the patient’s rights and responsibilities.
    • Maintaining patient privacy and confidentiality at all times.

     

    Qualifications:

    • Experience: Must not have had personal hospice services within the previous 13 months. Must be at least 14 years of age. If between the ages of 14-16, a work permit as required by applicable state law.
    • Skills: Must be able to read and write in English and follow written and verbal instructions in English effectively. Demonstrates interest in the Hospice philosophy.
