The Traveling Nurse - RN is a trained professional nurse who provides skilled nursing care to hospice and/or home health patients anywhere within the Great Lakes Caring service territory. This care is performed in accordance with the physician’s orders, under the direction and supervision of the RN Case Manager/Clinical Supervisor and within the RN scope of practice. The Traveling Nurse will receive temporary assignment from the Executive Director based on branch staffing needs. This position requires the ability to build rapport and communicate effectively with patients, families, physicians, facilities and other team members and the flexibility to be comfortable with adjusting to a different work and living environment on a consistent basis.

 

My commitment will include:

  • Making the initial evaluation and re-evaluating the patient’s nursing needs during each visit; Making follow up visits according to patient, family and facility needs.
  • Assuming primary responsibility for caseload, including assessing, planning, coordinating, implementing and evaluating the plan of treatment.
  • Possessing basic assessment skills specific to the provision of hospice and/or home health care, including but not limited to identification of home safety risks, appropriateness of home environment to the delivery of care, availability and capacity of support systems and community resources.
  • Calculating pain medication doses using equianalgesic conversion tables.
  • Continually assessing patient needs and obtaining data on physical, psychological, social and spiritual factors that may influence patient/family/caregiver health status and reporting findings to the Clinical Supervisor.
  • Making revisions to the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility and other team members
  • Counseling, instructing and including the patient in following the Plan of Care and meeting nursing and related needs.
  • Attending and actively participating in nursing facility care conferences for appropriate patient discussion, consultation and evaluation.

 

Qualifications:

  • Education: Registered Nurse, Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing
  • Licensure: Current State License as a Registered Nurse. Licensing required in all states served by Great Lakes Caring.
  • Experience: 2 years of nursing experience in a clinical care setting. Hospice or Home Health environment preferred.
  • Skills: Nursing skills as defined by accepted nursing standards. Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment. Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles. Ability to handle multiple priorities, documentation requirements and deadlines.
