The Traveling Nurse - LPN is a trained professional nurse who provides skilled nursing care to hospice and/or home health patients anywhere within the Great Lakes Caring service territory. This care is performed in accordance with the physician’s orders, under the direction and supervision of the RN Case Manager/Clinical Supervisor and within the LPN scope of practice. The Traveling Nurse will receive temporary assignment from the Executive Director based on branch staffing needs. This position requires the ability to build rapport and communicate effectively with patients, families, physicians, facilities and other team members and the flexibility to be comfortable with adjusting to a different work and living environment on a consistent basis.

 

My commitment will include:

  • Verifying the Interdisciplinary Plan of Care prior to each visit and providing care according to delegation by the RN, physician’s orders, assessment data and established standards and guidelines.
  • Incorporating patient care goals established in the Interdisciplinary Plan of Care into nursing visits, as evidenced by documentation on the electronic skilled nursing note.
  • Implementing the plan for patient safety, using the patient, family, facility and community resources.
  • Calculating pain medication doses using equianalgesic conversion tables.
  • Informing supervisor or designee of unusual or potentially problematic nursing facility or home issues.
  • Continually assessing patient needs and obtaining data on physical, psychological, social and spiritual factors that may influence patient/family/caregiver health status and reporting findings to the Clinical Supervisor.
  • Demonstrating proficiency in medication administration, including po, IM, SQ, SL, TD, PO, enteral tube, and IV routes.
  • Demonstrating proficiency in phlebotomy skills, including implementation of universal precautions and safe handling of sharps.
  • Attending and actively participating in nursing facility care conferences for appropriate patient discussion, consultation and evaluation.

 

Qualifications:

  • Education: Licensed Practical Nurse, Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
  • Licensure: Current State License as a Licensed Practical Nurse. Licensing required in all states served by Great Lakes Caring.
  • Experience: 2 years of nursing experience in a clinical care setting. Hospice or Home Health environment preferred.
  • Skills: Nursing skills as defined by accepted nursing standards. Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment. Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles. Ability to handle multiple priorities, documentation requirements and deadlines.
