The Bereavement Coordinator is responsible for coordination of the hospice bereavement services offered in the assigned territory.
My commitment will include:
Assessing on an on-going basis the bereavement needs of families, lead bereavement groups and the process of bereavement throughout the designated hospice territory.
Providing bereavement support through grief counseling, memorial services, family assessment and therapeutic group counseling.
Consulting interdisciplinary team, client and family in issues related to grief and loss.
Maintaining organized, effective and efficient systems and communication to ensure the continuity of quality patient care delivered.
Providing necessary skills to appropriately report changes and document pertinent information and care rendered to the patient to ensure continuity of care.
Education: Master degree in counseling, theology or related field, with documented training in Grief and Bereavement preferred.
Licensure: Must have valid driver’s license.
Experience: One (1) year experience in a health care or hospice setting. Experience in family therapy regarding issues of loss, grief, death and dying. Experience in developing and conducting programs of support.
Skills: Ability to develop and maintain rapport with family and clients. Excellent communication skills, both verbal and written. Ability to work well with an interdisciplinary team. Excellent skills in interpersonal relations and organization. Demonstrates problem solving and decision making abilities. Demonstrates knowledge of bereavement concepts; ability to relate concepts to personal experience. Knowledge of community resources and methods for client/family referrals. Ability to handle multiple priorities, documentation requirements and deadlines. Able to successfully execute professional presentations on behalf of the organization. Ability to document electronically within the organization’s software program.
