The Coder is responsible for selecting and recording the correct and appropriate ICD-10-CM codes to the OASIS data set items. This position establishes and maintains effective and efficient systems for the current ICD-10-CM coding guidelines.


My commitment will include:

  • Accurately converting the reason(s) for any procedures, services or supplies, performed or issued from written diagnostic statements that may include specific diagnosis, signs, symptoms and/or complaints into ICD-10-CM diagnosis codes.
  • Accurate use and knowledge of the ICD-10-CM coding manual which includes demonstrating knowledge of the CMS guidelines and fundamentals for coding.
  • Demonstrating accurate coding process of transferring written or verbal descriptions of diseases, injuries and procedures into numerical designations in the ICD-10-CM coding manual.
  • Accurately demonstrating the ability to sequence coding as the process of listing ICD-10-CM codes in the proper order according to the ICD-10-CM coding guidelines.
  • Accurately demonstrating the requirement for coding to the highest specificity, the highest number of digits possible, 3-7 when choosing an ICD-10-CM code.
  • Accurately demonstrating knowledge in the identification and use of Z-codes, specific ICD-10-CM codes to identify encounters for reasons other than illness or injury
  • Accurately demonstrating the ability in use and knowledge of the ICD-10-CM manual’s Volume 1 Tabular List and Volume 2 Alphabetic Index areas when locating and assigning codes.
  • Assisting in evaluating the agency operations and systems and identifies ways to enhance workflow and productivity.


  • Education: Associate Degree preferred.
  • Experience: 1-year experience in a health care setting.
  • Skills: Requires interpersonal skills and ability to communicate effectively with staff and take patient referrals. Knowledge of medical terminology required, Medicare and Medicaid guidelines preferred. Able to demonstrate problem-solving skills, organizational skills and attention to detail. Must have strong verbal and written communication skills. Ability to operate computer equipment, FAX and photocopy machines as necessary.
