    The Telehealth Nurse is a registered nurse or licensed professional nurse who monitors all telehealth client readings in accordance with the physician’s orders and under the direction and supervision of the Telehealth Supervisor.

     

    My commitment will include:

    • Assessing assigned case load of telehealth patients’ data to identify the physical, psychosocial and environmental needs of patients as evidenced by daily readings.
    • Calling patients whose readings fall outside established parameters to determine patient status and request retest if appropriate.
    • Reviewing retest results and notifying physician of any signs/symptoms and readings obtained.
    • Contacting physician or emergency services as appropriate if it is found that the patient needs emergency intervention.
    • Thoroughly documenting all interventions to “red alert” readings in all applicable software system.
    • Implementing all action steps able to prevent all unnecessary hospitalizations and emergency room visits.
    • Verifying transmission, assisting with set-up or troubleshooting with field staff, and setting alert limits in the software system.
    • Documenting and communicating all troubleshooting needs to the clinical coordinators, telehealth team and clinical supervisors as appropriate.

     

    Qualifications:

    • Education: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing. BSN preferred.
    • Licensure: Current State License as a Registered or Licensed Practical Nurse.

     

    • Experience: 2-3 years’ experience as a nurse in an appropriate clinical care setting or home health environment preferred.
    • Skills: Nursing skills as defined by accepted nursing standards. Requires interpersonal skills and ability to manage patient care episodes.
    • Transportation: Reliable transportation.
